Ask your treating GP, school nurse, case manager, aged care provider or support coordinator to click on the REFERRAL tab & fill in the requested information, & then we will send you a service agreement to complete online & allocate you an appointment. Services Provided
- Private Health (gap payment may apply please contact us for current fee schedule).
- Workcover or equivalant insurance claim.
- NDIS participants on a self or plan managed NDIS plan (NDIA managed participants are not able to be seen sorry). Areas of interest include mood disorders, ASD, burns, Acquired brain injury and Intellectual impairment. Or any physical impairment effecting upper limb function.
- Department of Veteran Affairs card holders. See your GP to refer you to Heartland Occupational Therapy using a DVA Request/ Referral Form (Form D904).
- Medicare: Chronic Disease Care Plan participants. The GP will send a written referral to us (Please note: $35.00 gap fee applies for short consult).

DVA veterans are billed through DVA using the schedule set by the DVA, so there is no out of pocket expense to the Veteran. Equipment is also paid for through the DVA schedule so there is no out of pocket expense to the veteran.
