Written referrals for intake consideration from Drs, NDIS Support Coordinators, Providers and Health Professionals are preferred.
Self-referrals are accepted for some self managed NDIS or HCP participants. To refer simply click on the REFERRAL tab on our website and fill in the requested information. If you are not sure what to do, please call and we can walk you through this process.

Please note: functional assessments are not offered unless funded through NDIS or HCP package.
Accepted Funding Options with no out of pocket expense
- Hand Therapy Workcover or equivalent insurance claims
- Home Care Package (aged care commonwealth package)
- NDIS for self or plan managed participants for Early Intervention or over 7’s with level 1-2 ASD, Arthritis, Anxiety, Intellectual Impairment and some other mood disorders (NDIA managed participants are not able to be accepted, but if we are your preferred provider, discuss options with your treating team to have your plan adjusted).
- Department of Veteran Affairs card holders not requiring major home modifications. See your GP to refer you to Heartland OT using Form D904 downloadable from the DVA website.
